Treatment response of ECT in depression

Electroconvulsive therapy (ECT) is mainly used in the treatment of depression. It often works better and faster than anti-depressants. However, so far, ECT is used as a last resort in the treatment program of depressive patients. Is there a role for ECT in an early stage of the treatment of patients who are likely to respond? How do we predict 'likely to respond'?
